MAPP is in the process of data acquisition for the current Community Health Needs Assessment. The more people in the community that we hear from, the better! If you or your organization are interested in helping to gather local data, please sign up here:

Please complete as much as you can, if you do not have answers to all of the questions, that is okay. A CHNA Committee Member will be in touch with you soon in order to arrange for whatever data acquisition tool is the best fit (survey, focus group, or interview).
